Improved Daily Living Skills (CB Daily Activity) – We provide comprehensive assessments and ongoing podiatry care.

NDIS participants, or individuals acting on their behalf can contact us to find out more information and arrange an initial NDIS podiatry consultation.

Before your consultation, we’ll need the following:

— Participant’s NDIS Number

— Copy of the participant’s NDIS plan or NDIS goals

— Funding details for: Improved Daily Living or Disability Related Supports

Fill out the Podiatry Services SA – NDIS Patient Referral Form in full.

Eligibility review, booking & payment

Once we receive, review and approve your referral and documents we’ll organise your appointment booking. Once the consultation is completed, we will claim payment via the NDIS Portal or through Plan Managers.
