Process for booking a home visit

New patient
We require a completed referral form – either NDIS or Home Care Package – provided by their Care or Support Coordinator. For private patients, our standard New Patient Form is sufficient.

Existing patients
If already on file with up-to-date details, we can transition them to Home Visits. However, if they’re switching funding types (e.g., from Private to NDIS or HCP), a new referral form must be completed by their Support Coordinator or HCP Provider.

Next steps
Once we receive the completed referral form, our Home Services Team will contact the patient to confirm any missing details, explain how our home visits work, and provide an approximate timeframe for their first appointment.

Podiatrist visit
One week before the scheduled visit, we will call the patient to confirm the day and approximate time of attendance.
On the day, the Podiatrist will attend as scheduled.

After your appointment
A tax invoice will be sent to the relevant funding body (Home Care Package Provider or NDIS Plan Manager).
The patient will be rebooked based on the Podiatrist’s clinical recommendation.
